Rachel Gulvin, PT, DPT
Welcome! My name is Rachel Gulvin. I’m a licensed physical therapist who is passionate about working with people with vestibular disorders, neurological disorders, and older adults. I earned my Doctor of Physical Therapy degree from Ithaca College in 2013. Over the years, I’ve completed extensive training, continuing education, and worked in several outpatient and hospital-based settings including the world-renowned Shirley Ryan AbilityLab in Chicago, where I’ve gained invaluable experience working with these populations.
I've received mentorship on Vestibular Rehabilitation, Functional Neurological Disorders, Neurological Disorders (like Parkinson's Disease and Stroke), and Concussion from the wonderful team at Re+Active Physical Therapy through their “Brain Bytes” program, the NeuroCollaborative mentorship program, as well as from Natasha Wilch through the “Concussion Nerds” program. I have also taken older adult specific continuing education courses through the Modern Management of the Older Adults program to better serve you.
I founded Bloom Physical Therapy in 2023 to serve people with vestibular disorders, neurological disorders, or older adults who are looking to regain function, return to meaningful activities, or rediscover joy in movement.
Bloom Physical Therapy currently offers outpatient physical therapy services to those in the Rochester, NY area either in our Penfield, NY clinic or through in-home mobile visits.

In-Clinic or In-Home Therapy
Bloom Physical Therapy offers both in-clinic and in-home outpatient physical therapy in order to best meet your individualized goals.
Our clinic is located in Penfield, NY. All clinic sessions are still private and one-on-one. The clinic setting allows us to utilize specialized equipment and safely perform interventions that may be difficult to perform in-home.
“Mobile” means that we can come to you. You do NOT need to be home-bound to qualify for our services. You no longer need to worry about driving in inclement weather, busy clinic atmospheres, driving while dizzy or light-headed, or finding a ride to and from appointments. Treatments can be more functional to meet your unique goals.
One-on-One Attention
At Bloom, all physical therapy sessions are always one-on-one (both in-clinic and in-home) and usually last for 45-60 minutes, which can be modified to meet your needs.
You won't be left alone to complete a cookie cutter program while your therapist assists other clients.
One-on-one care means that your treatment can be individualized to you, and your program can be updated, modified, or progressed when you need.
Specialized Care
Bloom Physical Therapy is NOT a general physical therapy clinic. We exclusively treat vestibular disorders, neurological conditions, and older adults.
Your therapist has years of experience working with vestibular and neurological disorders. She has worked at one of the top neurological rehab hospitals in the world, The Shirley Ryan AbilityLab, and has completed training and continuing education in these areas.
Bloom Physical Therapy also invests in specialized equipment for those with vestibular or neurological disorders to provide the best care possible. See below for more details.
Specialized Equipment
Infrared video goggles are an integral part of assessment and treatment of vestibular disorders. These goggles block out light and utilize infrared cameras to much better visualize even subtle eye movements and reflexes that provide information about the function of your vestibular system.
Only about 1/3 of eye movement due to vestibular issues are visible in room light. Infrared Video Goggles can reveal critical eye movements that would otherwise be missed.
These are available both in-clinic and during in-home visits.

The rehab arch allows you to safely work on balance, mobility, and functional exercises without the risk of falling. Not only can it act as a safety catch for falls, but it can actually provide unloading when needed. If you experience lower extremity weakness that makes standing and walking difficult, the rehab arch can provide extra support while you work to regain your strength. The rehab arch eliminates the fear and risk of falling, so that you can focus solely on improving your balance and function. This is great for older adults, those with balance issues, or those with neurological disorders.

Bloom Physical Therapy offers a comfortable and sturdy treatment table that not only moves up and down but can tilt in space. This is especially beneficial for those with BPPV who also experience neck or back pain or mobility issues. You no longer need to put your neck in painful or uncomfortable positions in order to perform BPPV treatments (such as the Epley maneuver). The entire table will tilt so that you don’t need to extend your neck.
If you’re having trouble performing BPPV maneuvers (such as the Epley maneuver) due to neck pain or stiffness, a tilt table will allow you to correctly complete the maneuvers and relieve your vertigo symptoms.
